How to Download, Save, and Print the Questionnaire:
Please click the link above and save a copy of the form to your computer.
You can do this by:
- Clicking the link
- Then look in the corners of your browser window for an icon that states “save” or “download.” (This will differ depending upon what browser you are using. For example: In Chrome the save icon is in the lower right-hand corner. In Firefox, it is in the top right-hand corner.)
- If you hover your mouse over the various options, you will notice a description of what that icon/option does.
- Find the “save” icon in this way.
- Once you have saved/downloaded the form, please fill out the requested fields as best you can.
- Save the filled-out document on your computer.
- The final step is to print the completed document.
